Consultation Symptom Questionnaire
You’ll use this form when prospective patients come in for a consultation. You’ll be able to tell, at a glance, if they have food or inhalant sensitivities. You might also use this form to supplement your own New Patient Information form if you choose not to use mine.
Sensitivity Care Program Costs
It was my firm belief that I should get rid of as many sensitivities from every one of my patients as I could. I could do this better after I developed packages and asked my patients to pre-pay for the package they needed or wanted. In return, I gave them a big discount. It was truly a win-win.
This form shows the patient what categories are in each package, how much the package would cost if they paid visit by visit, and how much they’ll save by purchasing the entire package in advance. It really made a difference in helping more people feel much better.
I’ve got the programs already laid out for you on this form. All you have to do is edit it to reflect your fees.
Session Record
This form is where you record the clearings you give your patients and helps you stay on track. Page 2 is a graph for tracking the patient’s symptom progress. Used with my Symptom Questionnaire, it’s a powerful tool to show your patient how much better they’re feeling compared to when they started care.
Testimonial Form
A simple form to edit and hand out to patients who’ve had good results. You’ll want to have copies on hand at all times.
New Patient Information
A four-page form you can use for all of your new sensitivity patients. It’s packed with sensitivity-related questions that help you get right to the core of their sensitivity problems and has been expertly typeset.
Patient Symptom Questionnaire
A form to use every five visits (or whenever you like) to help track your patients’ chief symptoms. I found that if patients started with seven chief complaints and had only one toward the end of their program, all they focused on was the one they still had. They forgot all about the six you helped get rid of. They won’t if you use this form every five visits and record the results on page 2 of their Session Record. You’ll have visual proof of their progress. More importantly, you'll be able to show it to them if (when) they complain about the one they're still experiencing.
